ALL APPLICANTS MUST SUBMIT:
  • STEER Application 2012 - DOWNLOAD FORM
  • STEER Application 2013 - DOWNLOAD FORM
  • Immunization Records - DOWNLOAD FORM
  • Letter of Good Standing (on institution letterhead confirming good academic standing)
  • Copy of your personal health insurance card, both back and front
  • $750 housing fee (made payable to UTHSCSA; click here for funding ideas)
VISITING MS4s NEED TO SEND ADDITIONAL FORMS:
  • All MS4s must FAX the STEER application to 210-567-4579, then submit all other documentation through the Visiting Students Application Service (VSAS). Payment for $750 can be mailed to UTHSCSA, STEER Program, 7703 Floyd Curl Drive, San Antonio, Texas 78229-3900. If you experience any difficulty downloading required documentation in the VSAS, please contact your institution representative listed on the VSAS website.
